Only if you want to receive mail delivered by the USPS. Not required if you only plan to receive express packages by DHL, FedEx and UPS
For a person or a company to receive Postal Mail through an CMRA (Commercial Mail Receiving Agency), he/she must sign the USPS Form 1583* in front of a representative of that agency, or have the form certified by a notary public and have it sent to the agency by mail along with two forms of identification.
The USPS Form 1583 authorizes the Agent to receive mail in behalf of the customer.
